Dear Parent, The Illinois State Board of Education invites you to participate in a school improvement survey for parents.
Please Note: This survey is NOT a component of the 5Essentials Organizing Schools for Improvement Survey. In this survey,
you will be asked to provide feedback about your child’s school. Please consider your child’s current experience at school
(i.e., 2024-2025 school year). This survey is designed to help school and district leadership improve their understanding of
the parent/school relationship.
Make sure you choose your child's school, not the District. The school's are under Dee-Mack not Deer Creek-Mackinaw.
