If you think your family qualifies for the Free & Reduced Lunch program, please fill out the Free and Reduced Eligibility Form. This program helps families with lunch costs and school expenses.

Please download and print the form if you have the capabilities, if not, please pick a form up at the District Office between 8:00-4:00. Please drop the completed form off at the unit office or put in the mail to Damon Hackett, 401 E 5th Street, Mackinaw, IL 61755.  The form must be signed therefore an electronic version cannot be used. 

Form completion: ONE form per household 

• Fill out area #1 with all family members living in the house and the school/grade for each student.
• Fill out area #2 if it applies otherwise leave blank.
• Fill out area #3 list all household members who are receiving any income with the amount and how often.
• Fill out area #4 with signature, printed name, and date (you do not need to put social security number)
• That is all that must be filled in and then return to the district office
