2008-2009 School Year Information
REGISTRATION
Registration for students in grades Pre-Kindergarten -
12 of Deer Creek-Mackinaw CUSD No. 701 is completed by mail for families
enrolled in the district at the end of the previous school year. All
registration material was mailed at the end of June and is due back to the
respective school offices by Monday, July 28, 2008. For families new to the district, or those
that did not receive registration information, please contact the appropriate
school office after Monday, August 4, 2008 to register. Due to
the new postal regulations, it is imperative that you update your address if
you no longer use a post office box.

Fees are to be paid at registration, whether via mail
or on site. Lunch payment may be
included with registration. Visa and
Master Card will be accepted. All
families who think they may be eligible for free/reduced lunches are encouraged
to apply. Physicals, vision, dental, and
immunization forms should be sent with registration.

Lunch fees may be paid at registration. Payments will be accepted for weekly, monthly
or semester. Payments are also accepted
online through the district website at www.deemack.org.

KINDERGARTEN
Children
must be five years of age by September 1, 2008 to enroll in kindergarten. Certified
birth certificates must be presented to the principal’s office on or
before the first day of school.
PHYSICAL EXAMS
Complete
physicals are required on or before the start of school for students entering
kindergarten, sixth grade (this is a
change from previous years), and ninth grades. If students do not have a physical, they must
have an appointment card from a doctor or dentist. A written note from the parent is not
acceptable. Exams must be dated within
one calendar year of entrance to school.
Athletic physicals do not satisfy the physical examination requirement
but the physical examination will meet the athletic physical requirement. All out-of state transfer students are
required to have physical exams.
The
Governor signed HB 5717 into law on Wednesday, July 16. This law states that a
student that received a health exam within one year prior to starting the fifth
grade in the 2007-2008 school year is not required to receive an additional
health exam when the student attends school for the 2008-2009 school year,
unless the student is starting school for the first time.
DENTAL EXAMS
Complete
dental examinations are required on or before the start if school for students
entering kindergarten, second, and sixth grades. Examinations must be performed by a licensed
dentist, and they must sign the proof of school dental examination form
(available in the school office).
VISION EXAMS
A
new state law requiring eye exams for all school aged children took effect on
January 1, 2008. The law requires
comprehensive eye exams for children entering kindergarten or enrolling for the
first time in public, private, or parochial elementary schools in
ATHLETIC PHYSICAL EXAMS / BIRTH CERTIFICATES/INSURANCE
All
student athletes, including cheerleaders and pom pons, must have a completed physical exam, birth
certificate, and proof of insurance or an insurance waiver on file in the principal’s
office before participation is allowed.
IMMUNIZATIONS
If
your child is not in compliance with the immunization requirements established
by the Illinois Department of Health, he/she will not be allowed to attend
classes on the first day of school, Wednesday, August 20, 2008.
FIRST DAY OF SCHOOL
The
first day of attendance for students in Deer Creek-Mackinaw CUSD No. 701 will
be Wednesday, August 20, 2008 with an early dismissal at 1:30 p.m. No student living within 1.5 miles of school
will be provided bus transportation.
EARLY DISMISSALS FOR STAFF DEVELOPMENT ACTIVITIES
School
will be dismissed at 11:30 a.m. on October 29, 2008, November 26, 2008, December
19, 2008, January 16, 2009, February 13, 2009, April 23, 2009, and May 29, 2009
for faculty school improvement activities.
Lunch will not be served on school improvement early dismissal
days. Staff development will take place
from 12:30 to 3:30 p.m.
EMERGENCY CLOSING
In
the event of an emergency closing, an announcement will be made on most local
radio and TV stations. Additionally,
parents will be notified using the district’s automated calling system.
EARLY CHILDHOOD SCREENING
All
children ages 0-5 living in Deer Creek-Mackinaw District #701 may be screened
for developmental levels in motor skills, general knowledge, social skills, and
speech and language by calling Ann Schwarting at
(309) 359-5480 and scheduling an appointment.
Screenings will be held on Wednesday, August 20, 2008 and Thursday,
August 21, 2008 from 8:00 a.m. to 4:00 p.m. at the Primary and Junior High
School.
