*A REMINDER TO INCOMING FRESHMEN

Dee-Mack High School will need a birth certificate with the raised seal for your student. Hospital forms are no longer sufficient. These certified birth certificates are available at the County Courthouse. We are aware of the time necessary for you to get these. We encourage you to get several, as your student will also need one for Driver’s Education. Please include the birth certificate with your registration.

Please disregard if you have previously provided your child’s grade school with a certified birth certificate.

Also, we will need a 9^th grade physical with up-to-date shot records. Td (Adult Tetatnus-Diptheria) is good for 10 years and is needed for most freshmen. Please make sure this is up-to-date. Our School Board has implemented the policy that students must have their physical and shot records up-to-date by the first day of school.

If playing a sport you need a sports physical before the first practice. Ninth grade physicals are good for sports for one calendar year. The sports physicals are not sufficient for the 9^th grade physical.
